Position: Transition Specialist

SUMMARY

 

Under the general supervision of the Direct of Federal Programs, the Transition Specialist will work under the direction of the Director of Federal Programs to ensure the educational stability of vulnerable students.  The Transition Specialist will facilitate and monitor the transition of DPS vulnerable students from vulnerable student populations (foster care youth, group home youth, and/or juvenile justice-involved youth) to the respective schools.  The transition facilitating and monitoring may include contact with students, parents/guardians, court officers, children services, school staff, and other support services.

 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Establish contact with students transitioning from Montgomery County Juvenile Center.
• Establish contact with students currently under the care of children services (i.e., foster care, group home).
• Support the identification, enrollment, and access to academic and non-academic services for students in foster care and group homes.
• Monitor student progress for transitioning successfully into or at their assigned schools.
• Establish procedures and protocols for communicating information to school officials, parents/guardians relative to progress of student transition and placement.
• Assist students in securing any needed additional social service support and social emotional support.
• Establish and maintain resource files of materials, contact persons, and information related to supporting the student’s successful transition to the learning environment.
• Compile and disseminate, under the direction of the Director, an end of the year summary of support provided to students.
• Maintain appropriate and required documents for Ohio Department of Education and Workforce federal programs monitoring for Title I, Part A, Title I, Part D (Neglected and Delinquent).
• Serve as a resource liaison between the student, school, and community.
• Establish relationships with relationships with Montgomery County Juvenile Center, children services, and other organizations in which students may transition from.
• Work with the juvenile justice center, local juvenile courts, community agencies and community partners to best support students in the juvenile justice system.
• Other responsibilities as assigned by the supervisor.

 

 

QUALIFICATIONS

• Master’s degree in a related field
• Minimum of three (3) years working with at risk students.
• Excellent written/verbal communication and organizational skills.
• Knowledge of community and district resources.
• Evidence of leadership experience and/or training.
• Expertise with software applications in Microsoft Office, Google Suites, and other applications.

 

 

PHYSICAL ATTRIBUTES/DEMANDS  

• Frequently required to sit, stand, walk, hear, see, read, speak, reach, stretch with hands and arms, crouch, kneel, stoop, and climb stairs
• Occasionally lift and/or move lightweight items up to 40 lbs.

 

 

WORK ENVIRONMENT  

• Work is performed in a classroom setting
• Occasional exposure to blood, bodily fluids, and tissue
• Work may be subject to frequent interruptions
• Some assignments are subject to completion within strict timelines
• Periodic travel for meetings, professional development activities, and work assignments
